I spoke with Ultra-Fab and they said if the jack was made before 2015 then there are no replacement motors available. The serial number on your jack starts with 12 which means it was made in 2012 therefore, your only option is to replace the entire jack. The upgraded model is the Ultra-Fab Electric A-Frame Jack part # UF38-944017 which has brighter LED lights, an external ground wire for more dependability, and a more secure cover for better protection from the elements.
