Question:
On a 12000 lb. equalizer hitch how much clearance should there be from the ground 3- 4 inches ok ?

When picking out a weight distribution shank the first priority is to go with one that allows the trailer to be towed as level as possible. The Pro Series shank part # 63911 can give you up to a 5-1/4 inch rise or a 2 inch drop.
If this shank is going to be sitting 3 to 4 inches off the ground you are probably going to have some ground clearance issues as you travel over elevation changes.
I attached a help article on picking out the correct rise/drop ball mount for you to check out as well.
