The Demco Hijacker Autoslide 5th Wheel Trailer Hitch w/ Slider, part # DM8550034 will work great with your 2004 Ram 2500, and will work with the Lippert pin box you have on your trailer. This hitch features an automatic slider that means you do not have to get out of the truck to operate the slider any more. It also features a wrap around single jaw that helps reduce rattle, and with a dual pivoting head you will have less chucking so that you have a smoother ride.
The 5th Wheel Rail Adapter, part # DM6099 you asked about is designed for Ram trucks that have the factory in bed puck system installed. The factory puck system was not available on Ram trucks until 2013 and will not work with this adapter.
For an above-bed rail system I recommend the Demco Hijacker Premier Series Above-Bed Base Rails and Custom Installation Kit, part # DM8552016-71. This system requires no drilling in the truck frame when installing and gives you an above bed set of rails for the fifth wheel hitch. For a system that requires drilling into the truck frame you can use part # DM8552015-71.
If you prefer an underbed installation, the Demco fifth wheel hitch you will need is part # DM8550022. This has the same capacities as the DM8550034, but is designed to work with the under bed rail system. The underbed rail and installation kit for this fifth wheel hitch is part # DM8551005. Drilling of the truck frame is required when installing this underbed system.
To keep the hitch head protected in the bed of your truck from the elements I recommend using a cover like part # DM14844. This is made of waterproof vinyl and is held in place with 2 included adjustable straps.
