Hello Doug, thanks for reaching out. The Trailer Valet Square Direct Weld Jack # TV57ZR does not come with a mounting plate. I went out to our warehouse and looked at one and it comes with just the jack itself - no mount.

What kind of trailer are you setting up this jack on?
