Hi Corey; the Ultra-Fab Steel Roller # UF48-979020 is designed to fit a 2" receiver. As long as you have a hitch with a 2" receiver on your 2021 Ford Transit it will fit.
What do you typically carry on the back of your Transit?

I use this wheel in the 2 inch receiver on the back of a 34 foot Arctic Fox 5th wheel trailer. Due to the distance from the rear axel to the rear of the trailer the original skid bars wore off due to the rear of the trailer dragging when I pull out of my driveway. This supports the back for the short distance it would drag.

Used it on the back of my 37" travel trailer to keep the receiver off the ground as my driveway is very steep and the trailer would bottom out. Installed the wheel assembly and problem was solved!
