Question:
In the photo provided, it shows the tube is visible below. It does not show if the box receiver extends past the bumper. My wife is concerned with hitting it with her leg.

I believe you are referring to the customer photo for trailer hitch # DT65MR. The receiver portion will stick out past the bumper on a 2014 Toyota Highlander. I have included an image from the instructions, which are also linked, showing the hitch's final position once installed.
The trailer hitches for the 2014 Toyota Highlander are currently all the same way. This is a legitimate concern. I have bumped my leg on a hitch and it is not pleasant! What I recommend is to put in a hitch cover to make the receiver more noticeable such as a chrome cover, # F-10P, or even something functional like a step, # CR-600. And use a hitch lock # e98880 to deter theft.
To complete your towing set up I recommend etrailer ball mount kit # 989900. It includes 2 ball mounts, 2 balls, a hitch pin with clip, and a storage bag. And for trailer wiring you can use # 118449. I have linked a video showing installation but a customer with a 2014 Highlander has told us that both connection points for the trailer wiring harness are at the rear of the vehicle instead of one being up front.
