Where Does the 4th Fifth Wheel Rail Bolt Attach to the Truck Since the Bracket only has 3 Tabs

Question:
The frame brackets only have 3-tabs for mounting the rails that have 4-mounting holes,where is the fourth bolt supposed to go?

The 4th bolt on each side of the bed bolts to the bed material and the hat channel underneath. The hat channel runs across both sides of the frame to support the bed. I have included a link to the instruction file for these brackets so you can get an idea of how they install.
