So does this only work on a flat bed, I have an 03 2500 Duramax with a b and w recessed in goose neck ball

Yes, it does but Demco makes an alternative. To attach to a gooseneck ball in a standard bed you will need the Demco Recon Gooseneck-to-5th Wheel Trailer Hitch Adapter item # DM8550045. This hitch adapter attaches directly to your gooseneck ball and will work with your 2003 GMC Sierra 2500.

The Demco Recon item # DM8550045 is made in the USA and has a 21,000 lbs capacity. It has height settings of 15-1/2", 17-1/4", and 18". It has a 3" offset as well and works with tall gooseneck balls up to 5".

The Demco Recon item # DM8550046 you were looking at is designed to be used in flat bed trucks that have a recessed goosebox installed. The base of this hitch is 22" x 22". The recessed well would need to be smaller so the base can sit on the flat bed.

To make towing with a 5th wheel easier I recommend adding the Curt 5th Wheel/Gooseneck Custom Wiring Harness w/ 7-Pole Connector - 10' Long item # C56000.
