The Blue Ox High-Low Adapter for Tow Bars # BX88235 is designed for lateral pulling like what we see with a flat-tow setup since the towed vehicle is supported on all 4 wheels.
For your application I recommend using the Replacement Shank for Gen-Y Adjustable Ball Mount # 325-GH-303 which offers the same rise/drop the Blue Ox does but this is more of a heavy duty piece that can be used for towing a trailer.
