The phrase 5 on 4-1/2 inch is the bolt pattern of the wheel. This means that there are a total of 5 lugs that hold the wheel to the hub and the distance across the hub from the center of one lug straight across is 4-1/2 inch. This is the bolt pattern that the part # AM3H390 you referenced has and is by far the most popular 5 lug bolt pattern.
