Will this work with a CM flatbed with recessed gooseneck ball hitch?

Demco makes a special version of the Recon hitch for flat bed applications that have recessed balls actually! So what you'd want instead is the part # DM8550046. This has a shorter overall height to make up for the additional height of the flat bed truck and has the portion that attaches to the ball sit a bit lower.

I purchased this produce to convert my goose neck to 5th wheel towing. I am having issues with the hitch plate turning in the bed of the truck. I re torqued to required specifications and next trip it moved again. It turns about 1" off center.

@DennisY Since that system just uses the 1 pin hole to secure the hitch ball I wonder if that pin hole has enlarged a little over time? Is the pin hole still circular or is it a little oblong?
