How far does this stick out from under my bumper? Ive got a 2019 4door JL

I don't know exactly how far the Draw-Tite hitch # DT78MR is going to stick out from your bumper but I can tell you that it's exactly 6-3/8 inches from the front of the mounting plate to the opening. The mounting plate itself is right at 3/4 of an inch thick so if you measure roughly 7 inches from the frame of your Jeep out past the bumper you can see where the opening of the receiver tube will be.

The size of the bolts that come with the Draw Tite hitch part #DT78MR are M12X1.75X120 but more importantly this does fit the Willys version of your 2021 Jeep Wrangler.
