I found some photos of the Heng's Roof Vent 122112 online. Based on this, it looks the correct replacement for you should be the Vent Cover for Ventline Old Style Rounded Dome Trailer Roof Vents part # BV0554-01 for white or part # BV0554-03 for black. In order to confirm this you would just need to make sure your roof vent has a continuous hinge (see photo). If you are not sure please just send me a photo of what you have and I will confirm. These covers are both 14-1/2" x 14-1/2".
