Question:
I want to install a 2 trailer hitch receiver on the back bumper of my 2018 Airstream 27FB to add a bike rack and or storage box. The frame is 67 inches wide. My question is should I attach the hitch to the 4-inch down bracket extensions from the frame or to the frame itself? If I attach to the frame do you have a hitch that will work or do the frame extensions need to be removed? See attached photo. Thanks,

The Ultra-Fab 2" Trailer Hitch Receiver for Trailer Frames part # UF35-946403 will need to be attached directly to the frame on your 2018 Airstream RV Flying Cloud 27FB, not the bumper support brackets. The warranty of the trailer hitch and bumper on your trailer would certainly be voided if you did not follow the above recommendation.
In regards to clearance, the Ultra-Fab will need to be positioned as far forward as possible to allow the receiver tube opening to be closer to the rear of the trailer. However, due to the large bumper on your trailer it looks like it will be recessed after installation but so long as you choose a bike rack with a long shank, a hitch extender should not be needed.
Please keep in mind that the bike rack must be rated for use on the back of a trailer. For a bike rack with a long shank and is rated for use on your trailer, I recommend the Swagman Trailhead 4-Bike Rack part # S63381.
