One more question, do these come ready to air up and mount or do I need to have them balanced first?

We have basically exactly what you are looking for with part # AM32352. This is a 5 on 4-1/2 inch conventional wheel with the tire size 205/75-15 mounted on it with a pilot diameter of 2.62 inch and can have baby moon covers like part # AM90086 installed.

What our tire manufacturers will do typically is line up the light spot on the tire with the heavy spot on the wheel. This is a type of balancing but not like how car tires are balanced (with weights on a balancing machine). You'd need to check the tire pressure and inflate to the max PSI listed on the sidewall of 50 psi.
