Does Demco GM Fifth Wheel Rail Kit Adapter Have to Face Specific Direction

Hi ! You guys have been great and I have the adapter in the truck ! One thing though and it has nothing to do with you guys is that I sure wonder about 2 ball bearings and nothing else holding this thing down ! I guess the Demco folks have made sure it works ???

I checked the install instructions for the Demco GM 8 ft bed fifth wheel rail adapter part # DM6206 and they show that the larger triangle on the top plate needs to be on the rear side of the truck bed.
For the short bed version of this adapter part # DM6194 the single triangle needs to face rear of truck.

Hello last year I purchased a Demco 5th wheel rail adapter for my 2019 chevy Silverado easy to install but I notice that twice it sheared the locking wing nut. is their a problem with the wing nut tinsil or something is not siting correctly inside the mechanism. can u let me know if the is just a fluke. and if I can can I order the wing nuts from u
