From my research your axle is a 3,500 pound, 2-3/8 inch diameter axle and since you have a 5 on 5 bolt pattern you likely have bearings # L44649 and # L68149. You'll want to verify this is correct with your current bearings.
If this is the case for a hub and drum assembly I recommend # AKHD-550-35-EZ-K which is for E-Z Lube axles with a zerk fitting. If you have standard axles use # AKHD-550-35-K instead.
If you only need idler hubs use # AKIHUB-550-35-EZ-K with E-Z Lube axles and # AKIHUB-550-35-K with standard axles.
All the hubs have a 5 on 5 bolt pattern and are for 3,500 pound axles.
For the u-bolts and plates you'll need U-Bolt Kit # APUBR-1.
