Do I have to use the plastic foot pad if I have a rubber bed mat

The included foot pads of that come with the B and W Companion Hitch # BWRVK3500-5W are designed to be used regardless if there is corrugation on the truck bed or not. The shape of them does allow them to fit in corrugation if needed which is nice. If you weren't to use them the hitch wouldn't make contact with the truck bed to support the load of the trailer. The only contact point would be with the gooseneck ball.
Since you don't have corrugation you will want to mount the pad with the flat, wider side down on the truck bed.
