Hi Brandon!

For the Dexter 5-Leaf Slipper Spring w/ Flat End for 10,000-lb Trailer Axles - 30-1/2" Long # DX45QR, you will want the Bronze Spring Eye Bushing, 3/4" x 1" x 2-1/2" # 14-67. This is a direct replacement for what came in this spring.

Guessing the original bushing wore out?

Most 10k trailer axles are 5 inches in diameter which is what the part #091148 is designed to fit. I'd be surprised if this did not work for you but I would still recommend you measure the diameter of your axle to be sure.
