Hey Jeff, since flat bed trucks sit up a little higher you would need to go with a low profile B and W Companion hitch part # BWRVK3050 which was designed for your setup. You will also need to have the Turnoverball gooseneck hitch part # BWGNRK1500 installed in your truck bed as well as this doubles as a rail kit for this hitch.
