What is the difference between these auto-adjust 7k axle drum brakes and the 23-464-465

The difference really comes down to brand recognition, price, and warranty, but apart from that they are very similar. Dexter is a leading brand and people know they are getting quality, hence the higher price. They Dexter also do have a 5-year warranty compare to the etrailer brand's 1-year warranty.
Going with the etrailer brand doesn't mean you're getting a bad set of brakes though; in fact, with thousands of customer reviews on both sets, each is rated very close to 5 stars. Really it just comes down to if you want the most economical option or are more comfortable going with the more familiar Dexter brakes. Both sets are self-adjusting and for 7,000 lb axles, and both do require the mounting hardware:
