Thanks for the picture! Your EZ Loader Model 5-TEZB 17-20/22 4000 trailer uses inner bearing item # L68149 and outer bearing item # L44649. The grease seal is item # RG06-050. You will need two of the Hub Bearing Kits item # BK2-100 for your trailer's axle as they are sold individually.
I have linked an installation video below.
I recommend using LubriMatic LMX Industrial Strength Grease item # L11390 to repack your bearings. You will want to grab one tube per axle.
