Based on your measurements, the MORryde 14-3/4" Shock Absorbing Equalizers for Tandem Axle Trailers w/ 42" Wheelbase # MR34AR will be a perfect fit for your trailer. Since you have 2-1/4" shackles I also highly recommend adding the MORryde Suspension Upgrade Kit # MR46ZR. This comes with wet bolts, bronze bushings, and 1/2" thick shackles that will last.
We have replacement axles and leaf springs here at etrailer, but I would need more information to find the right parts. For the axle you need the length which is the hub face to hub face measurement. In addition to the length, you also need to know the capacity. More than likely this information can be found on the axle tag.
For the leaf springs, you will again need the length and capacity. The length is the spring eye to spring eye measurement. If the leaf springs have gone flat you will need to account for that with your measurement since the springs would have originally been shorter. Also, leaf count does not matter and can vary within the same capacity springs so don't get caught up in matching it.
I have attached a diagram below, along with a helpful article on leaf spring suspension for your reference. I also attached links to all of our axles and leaf springs for you to check out.
