Was this item suppose to come with mounting hardware? If not what is the recommended grade and size bolts for this? Thanks

The Demco Cast Head Coupler eZ Latch # DM37ZR coms as the coupler only so you would need to supply the mounting hardware.

You will need 1/2" bolts that are at least Grade 5 rated. Anything higher than that will just be even stronger and work just fine.
