The Demco 5th Wheel Rail Adapter for Nissan Titan XD OEM 5th Wheel Towing Prep Package part # DM6200 requires that the locking plates be perpendicular to the bed sides for removal (reverse of installation). If the plate will still not budge once this is done there is likely rust inside the hitch ball opening and you'll need to use lube and or heat with force to remove it, there are no tricks really. I recommend trying to use the trailer jack to lift it out while still attached if possible.
Please note, you should be regularly applying grease to the opening if the adapter is left in the truck bed when not in use.
