Question:

Hello! I have a flatbed with a recessed gooseneck hitch. The ball is recessed five inches and has 6 inches of clearance on all sides. Will this adapter be able to connect? If not, do you have any suggestions?

I don't think you'll be able to use the Ranch Hitch Adapter # AM3139 for your situation because from the bottom of the gooseneck ball coupler to the bottom of the gray offset arm is about 4-3/4". From the top of the ball that changes to about 1-3/4". Then from the center of the center of the ball to the end of that offset arm is about 10-1/2".

For your application you're going to need a hitch that was built for a recessed ball application like the Demco Recon # DM8550046. The only issue with this adapter is that it doesn't have an offset for a short bed truck, but since you are using a recessed ball you are pretty limited in what you can use for a 5th wheel hitch.
