We have the Kodiak Disc Brake Kit - 12" Hub/Rotor - 6 on 5-1/2 - Dacromet - 5,200 lbs to 6,000 lbs # K2HR526D for your 5200 lbs axle. You will want to make sure that the bearing sizes match up for a confirmed fit but this is very likely the proper fit. I've attached an article to assist.

Then you will need a brake line kit for these calipers. I recommend the Kodiak Hydraulic Brake Line Kit - Single Axle - 18' # 18SI-BLKIT.

The last thing you need is a brake actuating coupler like the Dexter # 099-175-20. This electric lockout will be connected to your reverse light wiring so your brakes don't activate when you back up.

These parts will give you an excellent system that will work great together

I recommend the single piece hub and rotor over the rotor that fits over an idler hub. The reason is that it is one less moving part, so the rotors will track correctly with the bearings. The only benefit to the separate pieces are ease of replacement for the rotor as needed. Normally by the time you need to replace a rotor, the bearings are already needing replaced anyway, so I recommend you to with the single piece hubs like those that come in the # K2HR526DS we have already looked at.
