need a swivel pipe mount pipe jack with 3000 lb lift fully retrated height between 12 and 14 and 15 inches of lift

For your needs I recommend Bulldog jack # BD190708. It is rated higher at 5,000 pounds but all that means is that it will be easier on the jack. It fits all of the other criteria you are looking for. The fully retracted height is 12-5/8 inches and it has 15 inches of lift. It is a pipe mount swivel jack and comes with the proper size mating pipe that gets welded on.
