Can we tow a small boat with this setup

Curt trailer hitch # 13389 is rated for 3,500 pounds gross trailer weight and 350 pounds tongue weight (4,000/400 when used with weight distribution). If your 2005 Dodge Grand Caravan is rated as high then it can tow a boat/trailer up to the values specified above. If the van is rated lower you have to go by the van's rating. And if the van is higher you have to go by the hitch rating. Always go by the lowest rated component.
If you are not sure about the towing capacity of your van take a look at the picture I have included that comes directly from the vehicle's owner's manual.
I also recommend etrailer ball mount kit # EBMK4. It comes with a ball mount with a 2-3/4 inch rise or 4 inch drop, interchangeable ball set, hitch pin with clip, and a storage bag. And for trailer wiring I recommend # 118300 to add a 4-Way flat connector to the rear of the vehicle.
