I have a solution for you, but the CE Smith Offset Spare Trailer Tire Carrier # CE27310G that you referenced will not fit your bolt pattern.
The Ultra-Fab Folding Spare Tire Carrier # UF15-954730 will accept your 5 on 4-3/4 spare. This carrier fits a 4 inch square RV/trailer bumper. We tested this here once on a rim with your bolt pattern and found that it fit.
