Hey Seth, I am sorry but we have no hubs with any lug pattern that will work with your LM12749 outer bearing. That bearing is obsolete. The only solution I have for you is to replace your spindles with one like the item # TRU24FR so you'll be able to find a hub with modern use bearing sizes.
I have attached a diagram below that shows the dimensions of the #84 Spindle item # TRU24FR. If you could get some measurements off of your spindles I can match a replacement up as close as possible.
For a 5 on 4-1/2 hub that will fit a #84 spindle you'll want either the Easy Grease Trailer Idler Hub Assembly item # AKIHUB-545-35-EZ-K or the Galvanized item # AKIHUB-545-35-G-EZ-K if you've got a boat trailer.

Hey Seth, did you get a chance to measure your spindles to see if the #84 spindle was a close match? Once you confirmed that, were you able to move forward with one of the 5 on 4-1/2 hub assemblies? Your update could make it easier for others dealing with the same obsolete bearing issue.
