Hey Adam, thanks for reaching out!
For surface mount lights you need the Optronics LED Trailer Tail Light # STL78RB, but if by flush mount you mean recessed, then you need the Optronics Fleet LED Trailer Tail Lights w/ Grommets # TLL12RK.
Both kits come with 6-1/2" oval tail lights. I'm not sure what exact style came on your Heartland utility trailer, but either of these will be the same size as your 6-1/2" lights.
