Thanks! I am now leaning towards replacing the entire brake assembly. I see this one is not self-adjusting. What would be the equivalent self-adjusting brake assembly?

You're welcome Scott! For a 6,000 lbs self adjusting brake assemblies you will want the Dexter Nev-R-Adjust Electric Trailer Brakes item # 23-458-459.
If you need new mounting bolts and hardware item # BRKH12 will get you set up.
