Item # 8-201-9UC3-EZ This item shows 12 MTI on the hub. The one Im replacing shows 6 MTI. Is there a difference?

The 12 MTI or the 6 MTI that you are seeing on trailer hubs like the # 8-201-9UC3-EZ that you referenced is not related to the part number or fit so it wouldn't be something you'd need to match. If you see 8-201 on your hub and/or it's a 6k trailer axle then this for sure is the correct fit for you.
