Because there is no industry standard for wiring colors you will need to test the wires behind the taillights to determine their functions on your 2005 Chrysler Town and Country. To test the wires, you will need a circuit tester, # PTW2993. Have someone sit in the van and go through the lighting functions (each turn signal, running lights, and brake lights) while you test with the circuit tester. That will tell you which wires do what.
Or you could install a T-connector harness that just plugs in line with the vehicle tail light wiring, # 118300. No testing wires or cutting and splicing. Just plug it in, ground it, and you are done. I have linked a video showing a typical installation for you.
