As long as you don't have the 10" deep drop bumper on your 2004 Chevy Silverado 2500HD the trailer hitch you need to replace your OEM is the Curt Trailer Hitch Receiver # 13210. The design looks a little different but it still has a hidden crosstube and the bracket above the receiver like what you have on your hitch from the factory.
