My 2022 Highlander has 20 inch wheels. Is there a hitch that would allow me to retain the spare tire under the vehicle?

Most of the hitches available for your 2021 Highlander would prevent any spare tire larger than 18 inches from being stowed under the vehicle, but the exception would be the EcoHitch # EH35VR.
If you'll be towing with the hitch, you'll also need a trailer wiring harness like # TK99FR for a 4 pole connector or # TK57VR for a 7-way connector. For a ball mount, use # 2753 and # 19260 for a 1-7/8 or # 63845 for a 2 inch trailer ball.
