how does a Bargman 40-40-104 attach to a fender ? Is there a mounting bracket associated ?

Yes there is a base that comes with the trailer originally that the discontinued Bargman Trailer Fender Light part 40-40-104 is designed to fit into. On new installations you would add the base but it is not something we stock.
Instead the Optronics Thinline LED Trailer Fender Light for Trailers Over 80" Wide part # MCL65ARB and Black Bracket and Single Wire Plug for Thin Line Series Trailer Clearance and Side Marker Lights part # A65PB will make a complete installation kit for you.
