The Curt 3-Piece Hitch, part # C15511, is a confirmed fit for only one style of the 2006 Chevrolet Silverado 3500: the Cab and Chassis style.
There are two additional styles of the 3500: the Standard Pickup and the Pickup with a 10 Inch Deepdrop Bumper.
Unfortunately, if you have either of the Pickups, this hitch will not fit. Whichever you have, you can return the 3-Piece and get the correct hitch for your Silverado.
If you have the Standard Pickup, I recommend getting the Curt Class V, part # C15313. It's comparable to the 3-Piece but has a slightly higher gross trailer weight and tongue weight.
If you have the Pickup with a 10" Deepdrop Bumper, I recommend the B&W Heavy-Duty Trailer Hitch Receiver, part # BWHDRH25187. It's also comparable to the 3-Piece with a slightly higher gross trailer weight and tongue weight.
If you do in fact have the Cab and Chassis 3500, the reason the 3-Piece doesn't fit could be because of flexing. This can happen during shipping due to the material the hitch is made from. To remedy this, you can flex the brackets back by using a vice, a jack, or even just using a set of stairs. I've linked to an article that can help you with this.
