Question:

Will this fit a 12inch 3.5inch AL-KO brake setup? It looks similar and i know Dexter acquired AL-KO so was wondering if this part replaces the AL-KO 048-028 and 048-029.

I can't confirm that # 048-022-00 replaces the AL-KO adjuster number. Its designed to replace the adjustment screw on 10" and 12" Dexter Nev-R-Adjusting brakes.

However if you are needing to replace the adjusters on your 12" AL-KO brake assemblies the Replacement Brake Adjustment Screw Kit for 10" and 12" Electric Trailer Brake Assemblies # AKBRKR-AS-1012 will work perfectly since they are designed to work with several brake manufacturers including AL-KO.
