Do you air this up after you are hooked up to the truck?

Correct, once you have coupled the truck and trailer together you will then inflate the air bag inside the Reese Goose Box 5th-Wheel-to-Gooseneck Air Ride Coupler Adapter like part # RP85FR until there is at least 7" from the top of the truck bed to the bottom loft on the trailer.

What would be the pressure on air bag?

You'll want to start out with an air pressure around 35 to 40 psi, but the ideal pressure will depend on your specific setup. It's just a matter of adjusting the pressure from there depending on your personal ride comfort. The air bag does have a maximum 50 psi.
