Replacement Coupler Recommendation for Titan Model 20

Question:
Hello I was backing up my driveway and my trailer slipped of the ball. The trailer hitch has a tear in the metal behind the release lever. Titan model 20

How did the trailer become uncoupled? Was the latch not secured on the ball? If so then the coupler sounds like it needs to be replaced.
For that we have the Titan Model 20 as part # T4750720.
