does this products come with the brackets for Blue Ox or is that something that has to be purchased separately?

As it comes the Roadmaster Tow Defender # RM-4750 will work with Blue Ox tow bars connected to Blue Ox base plates. So provided you are using a Blue Ox tow bar and have Blue Ox base plates on your vehicle you would not need anything else.
