Will this base work with the older model 3000 18K lb hitch?

Hey Tony! The Replacement Base for B&W Companion OEM 5th Wheel Trailer Hitch for Chevy/GMC item # BWRVB3710 only works with other B&W Companion hitch heads like item # BWRVC3006. I am not sure what 5th hitch you're referring to but there were no 18,000 lbs capacity B&W Companions so the answer to your question is no.
If you are referring to the B&W Patriot item # BWRVK3255 the only way to convert this hitch to fit the puck system in a 2024 Sierra 3500 is to use the Adapter Rails for Curt 5th Wheel Hitch item # C88KR. This adapter will work most 5th wheel hitches, except for the Patriot and Husky slider hitches, so long as your current one attaches to industry standard rails.
If you're wanting to use a Companion hitch that mounts directly to your Sierra's puck system you'll need the item # BWRVK3710 for a 20,000 lbs capacity or item # BWRVK3715 for a 25,000 lbs capacity.
