I have a 2000 dodge 3500 with a wooden decked flatbed. There is an existing goose neck ball in the bed, will this adapt to that configuration? The description and customer photos werent necessarily clear on how the fifth wheel actually mounts to the truck.

No, the B&W Companion 5th Wheel Trailer Hitch # BWRVK3500-5W is only designed to work with a custom underbed mounting kit like the Custom Underbed Installation Kit for B&W Companion 5th Wheel Trailer Hitches # BWGNRK1394-5W.
I'd recommend going with the B&W Turnoverball Underbed Gooseneck Trailer Hitch for Flatbed Trucks # BWGNRK1500 instead, and use the B&W Companion Flatbed Gooseneck-to-5th-Wheel Trailer Hitch Adapter # BWRVK3050.
This will swap the current Gooseneck hitch out with the B&W Turnoverball while simultaneously allowing you to use their Companion hitch giving them the both options while not breaking the bank.
