Recommended Spacing for Mounting Blue Ox Heavy-Duty Universal Bolt-On Adapter Tabs

Question about the BX88266 bolt-on adapter tab: What fastener size is used for this? We are bolting it to a 4X4X 0.25 thk wall square tube and want to consider using RIVNUTS to fasten tab to the tube or weld a stiffener plate to the tube but want to specify fastener hole locations and use tapped holes. Is there a preferred horizontal spacing between the plates too?

I reached out to Blue Ox and found that the Blue Ox Heavy-Duty Universal Bolt-On Adapter Tabs - 20,000 lbs # BX88266 should only be mounted with the original hardware. For the recommended spacing, they say it should be 2” up and down and 5-1/2” side to side center hole to center hole.
