will this product fit a 2004 Chevrolet 2500hd short bed? If so how would i need to install it?

The Curt gooseneck plate part # C65500 that you referenced is a universal gooseneck hitch solution that could fit your 2004 Chevy Silverado. To install it, you will need the Custom Installation Kit for Curt Overbed, Folding Ball Gooseneck Trailer Hitch, part # C61341. You will need to drill holes and provide your own hardware as explained in the attached installation instructions.
We have easier solutions that won't require frame drilling for your truck as well like with the # C630-652 which has a removable ball too so that when not needed your truck bed would be unobstructed.
