Does the item listed below only work on a Nissan Titan truck? Will this work on a 2018 RAM 3500?

It looks like you're trying to get a good sized hitch extender for your 2018 Ram 3500. The Titan Hitch Box Extension for 2-1/2" Trailer Hitches, 41" - 48" Long part # RP45018 is designed to fit any 2.5" x 2.5" hitch it then reduces the opening at the end to 2" x 2" for use with many different accessories. While I did not see any specific information regarding an exact figure you are going to loose hitch capacity when you bring the extension out that far without somehow adding some additional bracing. The torque factor is drastically increased here.

A second alternative that I really like and is rock solid every time when trying to accomplish this is the TorkLift system. This system adds an additional hitch to your Ram then a dual arm extension that is the best in the business which allows you to tow and haul with no sacrifice in weight capacities.

For the hitch use the TorkLift SuperHitch Original 20k Trailer Hitch Receiver part # TLD1108 or the big boy TorkLift SuperHitch Magnum 30k Trailer Hitch Receiver part # TLD1108-30.

Once installed yo can choose the length of TorkLift SuperTruss Hitch Extension that you want for example part # TLE1548 will give you a 48" extension. I will also link the other options available and videos on the other recommendations to check out. The TorkLift Wiring Harness Extension for 48" SuperTruss part # TLW6048 is also a good idea if you'll be hauling a trailer.
