I have an 18 ram 2500 with factory gooseneck. Can I use a bandw offset on this for the shortbed?

Yes, the B&W Extend-A-Goose Gooseneck Coupler # BWTEXA4200 that you referenced will work great with the factory gooseneck ball on your 2018 Ram 2500. This is a perfect solution for creating the needed cab clearance during sharp turns, especially since there isn't an offset ball available that fits the factory puck system on your truck.
