The nuts on the factory brakes had no lock washers. Should that be?

Not necessarily. Some brake assemblies have Loctite like part # LT37423 on the bolts while other assemblies, like part # AKEBRK-7L-SA, require mounting hardware that includes lock nuts like what you would find in the kit # BRKH12S.
If your brakes have Loctite on the mounting bolts then you don't really have to worry about the lock nuts on them.
